The upload link you received was generated by a General Contractor who has requested proof of insurance and tax documentation from your company. Documents you submit may include a certificate of insurance, a workers' compensation certificate, and IRS Form W-9, along with the policy expiration date and coverage limits you enter.

By submitting a document, you consent to the requesting General Contractor and to Compliance Desk storing, reviewing and verifying the insurance and tax information contained in it for compliance tracking purposes, and to receiving renewal reminders at the contact details on file for you or your insurance agent.

Only the General Contractor who sent your link, and authorized platform personnel maintaining the service, can access your submission. Documents are held in private storage and are not published, listed publicly, or sold.

Submit only documents your company is authorized to share, and confirm the details you enter are accurate. Compliance Desk does not verify, endorse or guarantee the validity or adequacy of any policy; that determination remains with the General Contractor and their licensed insurance agent.

To correct or withdraw a submission, contact the General Contractor who requested it. They can remove or replace the record in their compliance roster.
